Since we focus on what we do best, you benefit from knowing that your therapist has the experience and skill to correctly identify your problem area(s). Your therapist then correlates those finding with your needs and desires, and develops a plan with you for achieving your goals for recovery in a timely manner.
1. Arrive 15 minutes prior to your scheduled appointment. You have several forms to fill out and sign.
2. New Patient Paperwork: We ask that you complete several forms, 3 of which are conveniently available online: SAPT Express Registration Form, Pre-Exam Questionnaire and ________ Pain Form (specific to your condition). You can actually do this at home. Look on the right side of this page and click on “New Patient Forms.” Click on the appropriate form, print it and fill it in by hand. Don’t forget to bring the completed forms with you to your first visit. If you don’t have this capability, don’t worry. We can take care of it all in our office on your first visit.
3. Know your insurance benefits and your out of pocket responsibility. If you have not met your deductible and/or if you have a co-pay or co-insurance please be prepared to pay at the time of service. Click on “Insurances” at the top of the page for more details.
4. Wear something comfortable that can allow you to move freely and expose your painful area.
5. Be prepared to spend 30 minutes to 1 hour with your Physical Therapist. You will be asked questions and your condition comprehensively evaluated. You will likely be issued exercises and other treatments may be performed as you and your Physical Therapist decide necessary.
6. Upon completion of therapy, you will check out and make follow up appointments as you and your therapist decide are necessary to best meet your needs for full recovery.
